Hospital Nights vs. Inpatient Days: Check Medicare SNF Coverage
Verify hospital status before counting on skilled nursing coverage.
With Original Medicare, an overnight hospital bed does not establish inpatient status. Check the recorded status before planning for skilled nursing facility care after discharge.
Confirm status, not just nights
Medicare’s hospital-status guide says inpatient status begins with formal admission under a doctor’s order. Observation remains outpatient care. Ask the hospital or patient advocate about your status each day.
If outpatient observation lasts more than 24 hours, the hospital must give you a Medicare Outpatient Observation Notice (MOON). It explains observation status and possible cost consequences.
Illustrative record: observation Thursday, formal inpatient admission Friday, discharge Monday. The inpatient days are Friday, Saturday and Sunday; Thursday and Monday do not count. This is a status timeline, not a coverage determination.
Check every SNF coverage condition
Medicare’s SNF coverage guide generally requires a medically necessary inpatient hospital stay of three consecutive days, excluding discharge day. Emergency-room and observation time before admission do not count. Ask whether an approved ACO three-day-rule waiver applies.
Other conditions still matter: Part A coverage and benefit-period days remaining; SNF entry generally within 30 days; daily skilled care ordered by your provider; and a Medicare-certified facility. The care must address a condition treated during the qualifying hospital stay, or a new condition arising during SNF care for that condition.
Skilled care can maintain a condition or prevent deterioration, not only improve it. Ask staff to confirm coverage and your expected costs; the day count alone is insufficient.
If inpatient status was changed
Eligible Original Medicare patients reclassified from inpatient to outpatient observation can seek a status-change appeal. Ask whether the Medicare Change of Status Notice (CMS-10868) applies and request it if missing. Being an outpatient from arrival is not the same as having inpatient status changed.
Follow the notice’s instructions promptly. Medicare recommends seeking review while still hospitalized when possible. Contact the listed Beneficiary and Family Centered Care Quality Improvement Organization (BFCC-QIO), checking that it serves your state. If already discharged, do not assume appeal rights disappeared; contact the QIO about the applicable process.
A favorable status decision may support SNF coverage only if the other requirements are met. It does not guarantee a free stay.
Medicare Advantage costs and coverage may differ; check your plan.
This guide does not recommend changing treatment or extending a hospital stay. Historical retrospective appeals are outside its scope. Checked October 24, 2026.
