Medicare Premium Tiers

Compare monthly charges.

Premium comparison.

Social Security’s 2026 chart assigns Medicare Part B premiums by income tier. This is not a tax percentage applied only to income above a threshold.

For this purpose, modified adjusted gross income (MAGI) combines adjusted gross income and tax-exempt interest. The 2026 determination generally uses 2024 tax information.

Compare three fictional single filers. Assume those tax-year figures govern, each has twelve months of Part B coverage, and no other premium adjustments apply.

At $109,000 MAGI, the standard monthly premium is $202.90. At $109,001, the first higher tier adds $81.20, making $284.10 monthly. At $120,000, that same monthly amount applies; this tier extends through $137,000.

The modeled annual difference between the first two people is $81.20 × 12 = $974.40, despite their $1 income difference. The third person does not pay another surcharge for each additional dollar within that tier.

Check the determination’s income year, filing status and coverage months before budgeting. This comparison excludes Part D and medical bills; it neither calculates total healthcare costs nor recommends changing income.
